Patient Support Representative - Family Medicine

Duties/Responsibilities:

  • Performs appropriate screening when patients and visitors enter the facility, in accordance with current infection control guidelines as assigned.
  • Communicates with patients and their families in a courteous, professional, cooperative and mature manner both in-person and over the telephone.
  • Answers telephone, responds to messages/inquiries and routes calls in a timely and polite manner.
  • Assists patients with questions as appropriate, or refers to appropriate person for assistance.
  • Assists patients in preparation for telephonic and virtual appointments, when such appointments are necessary.
  • Responds to patient requests for scheduling and rescheduling appointments to include creating profiles for new patient looking to establish care.
  • Schedules appointments utilizing the designated templates in the Electronic Medical Record system, or other supplemental software programs, as appropriate.
  • Manages patient portal requests via Well Messenger.
  • Assists with arranging language interpretation services, if necessary.
  • Fosters a patient-oriented environment that focuses on patient satisfaction and quality outcomes/monitoring.
  • Assist with clerical duties such as obtaining medical records, printing out medical records, printing out orders, printing out patient education, making discharge packets for patients, updating charts, labeling medical records, etc.
  • Cleans and prepares exam rooms in accordance with infection control guidelines.
  • Completes supply stocking as necessary.
  • Assists with patient flow throughout the office.
  • Assists patients with accessing Tandem Health technical resources, such as patient portal, satisfaction surveys and pharmacy mobile app.
  • Assists in providing PI reports for improving the quality of patient care.
  • Performs other duties as assigned by supervisor.
  • Manages appointment slots appropriately to allow for work-in patients in the daily schedule.
  • Documents communication with patient/family in electronic medical record, when needed.
  • Arranges for a callback from the provider or nursing staff regarding a patient's condition or question, as appropriate.
  • Works as needed with front desk and clinical teams for any scheduling needs that arise.
  • Maintains patient confidentiality in all encounters.

Qualifications:

Education and Experience:

  • High School Diploma or equivalent.
  • 1-year of healthcare experience preferred.
  • Certificate from a Patient Care Tech or Certified Nursing Assistant program desirable.

Knowledge, Skills and Abilities:

  • Computer knowledge and typing skills required
  • Ability to be cross-trained in reception, appointment scheduling, and general clerical functions

  • Ability to establish rapport and deal tactfully with persons from varied racial, ethnic, cultural and/or economic backgrounds
  • Ability to learn and correctly use Electronic Medical Record software program
  • Ability to remain calm and professional in stressful or emergency situations
  • Possess the ability to prioritize tasks and manage time efficiently
  • Ability to adapt to changes in the work environment, and is able to deal with frequent change, delays or unexpected events
  • Ability to maintain a high level of confidentiality
