The Community Health Worker (CHW) will be responsible for assisting patients to navigate and access community services and adopt healthy behaviors. The CHW supports the Primary Care and Population Health Providers and staff through an integrated approach to care management and community outreach to improve patient care and outcomes. 

The Licensed Clinical Psychologist provides a full range of psycho-diagnostic services including psychological testing of patients.

The Clinical Director of Behavioral Health is responsible for providing clinical leadership, fostering superior quality patient care, developing clinical strategic goals and direction, and assisting with the recruitment and retention of Behavioral Health Providers.
