The Registered Nurse (RN) Care Manager is responsible for providing medical case management services through education, counseling, patient advocacy and coordination of services to patients of Tandem Health.


The Nurse Site Manager will provide oversight and leadership for Pediatric services. The Nurse Site Manager will work closely with the Director of Clinical Operations, Providers and other professional and ancillary staff to provide effective leadership for the clinical team and to ensure that the department consistently provides high quality care with positive patient outcomes. He/She must have a strong nursing clinical background and be able to make decisions based on the situational presentation of patients and/or issues affecting patient care and flow.


The Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant is responsible for the diagnosis, treatment planning, and medication management of assigned patients in an outpatient behavioral health practice.


The RN performs a variety of direct and indirect patient care activities under the supervision of the Clinical Manager or Provider. Administers skilled nursing care to patients according to their physical and emotional needs and in accordance with the medical plan of care to include assessing, planning, implementing, and evaluating the care of all assigned patients in the center as appropriate
