The IT Operations & Support Specialist provides advanced system-wide Information Technology support to all users to effectively manage Tandem Health’s computer systems, infrastructure, and enterprise applications. This position serves as a senior technical resource responsible for troubleshooting complex issues, supporting infrastructure and security operations, assisting with technical projects, and ensuring stable day-to-day IT operations that support organizational growth and patient care.
This role also assists with employee onboarding and offboarding processes, user account administration, technical training, and coordination of IT-related initiatives across departments.

Responsibilities include precepting the Nurse Practitioners on staff and assuming an active role in meetings, provider CPI activities and other team building activities.

Provides direct care to patients in the primary care setting. Performs medical examinations, diagnoses, treatment, follow-ups, consultations, and health education within the scope of expertise and agreed upon protocols. Consults preceptor(s), as appropriate, if the health status of a patient is beyond his/her scope of practice. Assists in providing continuous process improvement reports for improving the quality of patient care.

The Registered Dietitian will work collaboratively within the multi-disciplinary team to create desired goals and outcomes of nutritional care for patients in all practice sites. The Registered Dietitian is responsible for providing comprehensive assessment, care coordination services, disease education and self-management support with patients who have chronic health conditions, specifically but not limited to diabetes, hypertension, and obesity. The Registered Dietitian will be integrated into the office-based health care team to work in partnership to promote patient-centered care, frequent contact with primary care providers, medical home team members, and actively participate in interdisciplinary patient-centered team meetings.

The Pediatric Social Worker is responsible for providing trauma-informed social work services through psychosocial assessment, counseling, care coordination, patient advocacy, and linkage to community resources for children and families engaged in the Pediatric ACEs Learning Collaborative. The Social Worker supports the implementation of evidence-based approaches to reduce toxic stress, enhance protective factors, and improve pediatric health outcomes within the collaborative.
