RN/LPN - Triage Nurse-Pediatrics

Duties and Responsibilities:

  • The triage nurse will be part of a care coordination team supporting general care processes alongside the providers.
  • The triage nurse will manage nurse-directed patient telephone calls and from walk-in patients requesting health care services.
  • Evaluates and assesses patient’s health care needs based on patient’s signs and symptoms utilizing established triage protocols and guidelines.
  • Obtains appropriate input and direction from the provider as needed to determine best clinical course of action and recommendations for specific patient health care needs.
  • Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response. Follows policies, procedures, and protocols to ensure consistency and departmental effectiveness as well as improve health care outcomes of patients/callers and their access to appropriate health care.
  • Coordinates and schedules same-day walk-in appointments on the provider schedules.
  • Utilize standing orders, triage protocols and algorithms appropriately.
  • Assures that pertinent medical information is forwarded to proper departments as needed for continuity of patient care.
  • Documents in electronic health record (EHR), triage assessment, plan, implementation, and evaluation, follow-up
  • Provides triage decision making where needed and under the guidance of the supervising provider.
  • Administers skilled nursing care to patients according to their physical and emotional needs and in accordance with the medical plan of care to include assessing, planning, implementing, and evaluating the care of all assigned patients in the center as appropriate.
  • Responds to callbacks as directed by the provider.
  • Completes prior authorizations as needed.
  • Completes other duties as assigned by supervisor.
  • Assists with precepting new clinical employees and student nurses.
  • Assists with nursing procedures as directed by the supervisor or providers.
  • Assists in patient/family education for continuation of care.
  • Observes, records and reports all significant information, signs and symptoms to the provider.
  • Performs triage expectations as appropriate to specific populations.
  • Collects specimens by proper procedure (urine, stool, wound, throat cultures, etc.).
  • Assists patients to prepare for examination or treatment and also assists MD/NP during examinations of treatments.
  • Performs EKG’s upon the request of the provider.
  • Assists in providing process improvement (PI) reports for improving quality of patient care.
  • Administers medications and monitors patients for adverse drug reactions in the center.

Qualifications:

Maintains a current license as a Licensed Practical Nurse or Registered Nurse with the South Carolina State Board of Nursing. A minimum of two (2) years’ clinical experience required. Current CPR certification. Must have good written and verbal communication skills. Must be able to relate to a variety of people at different developmental levels. Must possess the ability to prioritize tasks and manage time efficiently. Must be flexible as to the changing needs of the center. Must be able to work as a team player at all times.
