The Pharmacist is responsible for providing pharmaceutical services in accordance with policies and standard operating procedures set by the Corporation and within the constraints of the South Carolina Board of Pharmacy. The pharmacist performs duties under the direct supervision of the Director of Pharmacy Services and is responsible for compounding and dispensing medications, ordering, receiving and storing pharmaceutical supplies; maintaining drug utilization records, consulting with health care professionals (providers/ancillary) and analyzing patient drug profiles.
Provides direct care to patients in the primary care setting. Performs medical examinations, diagnoses, treatment, follow-ups, consultations, and health education within the scope of expertise and agreed upon protocols. Consults preceptor(s), as appropriate, if the health status of a patient is beyond his/her scope of practice. Assists in providing continuous process improvement reports for improving the quality of patient care.
The Registered Dietitian will work collaboratively within the multi-disciplinary team to create desired goals and outcomes of nutritional care for patients in all practice sites. The Registered Dietitian is responsible for providing comprehensive assessment, care coordination services, disease education and self-management support with patients who have chronic health conditions, specifically but not limited to diabetes, hypertension, and obesity. The Registered Dietitian will be integrated into the office-based health care team to work in partnership to promote patient-centered care, frequent contact with primary care providers, medical home team members, and actively participate in interdisciplinary patient-centered team meetings.

The Family Medicine Physician provides direct care to patients of all ages in a primary care setting. Performs medical services including examinations and evaluations, diagnosis, treatment, follow-up, consultations, and health education to patients of all ages within the scope of family practice. Responsibilities include precepting the Nurse Practitioners on staff and assuming an active role in meetings, provider CPI activities and other team building activities.
The Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant is responsible for the diagnosis, treatment planning, and medication management of assigned patients in an outpatient behavioral health practice.
The RN or LPN performs a variety of direct and indirect patient care activities under the supervision of the Clinical Manager or Provider.

The Nurse Site Manager will provide oversight and leadership for Family Medicine services in Sumter. The Nurse Site Manager will work closely with the Director of Clinical Operations, Providers and other professional and ancillary staff to provide effective leadership for the clinical team and to ensure that the department consistently provides high quality care with positive patient outcomes. He/She must have a strong nursing clinical background and be able to make decisions based on the situational presentation of patients and/or issues affecting patient care and flow.
