Behavioral Health Counselor - LISW or LPC
Duties and Responsibilities:
- Provides counseling and support to individuals and families experiencing behavioral, emotional, mental health, substance use, and psychosocial challenges.
- Develops and implements individualized treatment plans using current behavioral health assessment, diagnostic, counseling, and treatment techniques.
- Counsels clients individually and/or in group sessions (as appropriate) to assist the client in overcoming individual or family issues, mental illness, or substance abuse.
- Monitors patient progress, evaluates treatment effectiveness, and modifies treatment plans as clinically appropriate.
- Counsels family members (as appropriate) to assist with treatment issues and to serve as a support system to the client.
- Refers clients to other supportive services as needed.
- Prepares and maintains reports and case histories.
- Assists in providing quality assurance and process improvement reports for improving quality of patient care and provides support to the Case Management Team, as needed.
- Maintains timely, accurate, and complete documentation in the electronic health record (EHR) in accordance with organizational policies and regulatory requirements.
- Performs other duties as assigned by supervisor.
Qualifications:
Education/Experience:
- Master's degree in Social Work or Counseling required.
- Current, unrestricted South Carolina licensure as a Licensed Independent Social Worker-Clinical Practice (LISW-CP) or Licensed Professional Counselor (LPC) required.
- One to three years' experience in a primary healthcare setting preferred.
- Must maintain a South Carolina Driver's license
- CPR certification required.
