The Human Resources Officer is responsible for the planning, organization, and oversight of all activities of the Human Resources department. The Human Resources Officer provides support and leadership to all managers and employees of Tandem Health.

The Family Medicine Nurse (RN/LPN) performs a variety of direct and indirect patient care activities under the direct supervision of the Nurse Site Manager.

The Pediatrician provides direct care for children from birth to adolescence in a primary care setting. Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultations, and health education for patients within the scope of expertise. Responsibilities include precepting the pediatric Nurse Practitioner and assuming an active role in meetings and other team building activities. The Certified Medical Assistant assists in patient care, patient lab work, patient and laboratory record keeping and office and lab maintenance under the direct supervision of the health care provider and licensed nursing staff.

The Clinical Director of Behavioral Health is responsible for providing clinical leadership, fostering superior quality patient care, developing clinical strategic goals and direction, and assisting with the recruitment and retention of Behavioral Health Providers. Delivers effective and efficient patient care during required clinical time in compliance with established clinical protocols.

The Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant is responsible for the diagnosis, treatment planning, and medication management of assigned patients in an outpatient behavioral health practice.

Administers skilled nursing care to patients according to their physical and emotional needs and in accordance with the medical plan of care to include assessing, planning, implementing, and evaluating the care of all assigned patients in the center as appropriate.

Under the direct supervision of the Pharmacy Technician Supervisor and the Director of Pharmacy, the PRN Pharmacy Technician will assist the Pharmacist in performing all routine tasks and in managing and coordinating all aspects of the Pharmacy Department in accordance with established policies and procedures set forth by the Tandem Health and within the constraints of the State Board of Pharmacy.

Under the direct clinical supervision of the assigned Dentist and the Dental Services Clinical Coordinator, the Dental Assistant is the dentist's chair-side helper who assists with patient care, as required. The Dental Assistant may also act as a front office dental receptionist.

The Women’s Health Registered Nurse (RN) Case Manager/Educator will develop, implement and manage educational and support programs that promote women’s health in addition to facilitating evidence-based guidelines to educate team members, patients and the community. The RN Case Manager/Educator will coordinate and guide patients and families throughout the continuum of their pre-pregnancy and post-partum experience to include contraceptive care services. The RN Case Manager will be integrated in to the office-based health care team to work in partnership to promote patient-centered care, frequent contact with primary care providers and medical home team members, and actively participate in interdisciplinary patient-centered team meetings. The RN Case Manager will also work as part of the clinical support team to provide direct patient care as needed.

The pharmacist performs duties under the direct supervision of the Director of Pharmacy Services and is responsible for compounding and dispensing medications, ordering, receiving and storing pharmaceutical supplies; maintaining drug utilization records, consulting with health care professionals (providers/ancillary) and analyzing patient drug profiles.

Responsibilities include precepting the Nurse Practitioners and Physician Assistants and assuming an active role in meetings, provider CPI activities and other team building activities. The Behavioral Health Counselor provides School-Based Health Care (SBHC) behavioral health services to students and their families at assigned schools in Sumter County. The SBHC Behavioral Health Counselor partners with school administrators, faculty and school nurse to provide high-quality, family-centered care. The SBHC Behavioral Health Counselor provides mental health assessments and treatment plans. SBHC services are coordinated with the students, their parents and other health care providers.

The Registered Nurse (RN) Care Manager is responsible for providing medical case management services through education, counseling, patient advocacy and coordination of services to patients of Tandem Health.

Provides direct care to patients in the primary care setting. Consults preceptor(s), as appropriate, if the health status of a patient is beyond his/her scope of practice. Assists in providing continuous process improvement reports for improving the quality of patient care.
